Diabetic Cake Nutrition and Exercise

Medical nutrition therapy is recommended for all patients. For individuals with type 1 DM, the focus is on regulating insulin administration with a balanced diet to achieve and maintain a healthy body weight. A meal plan that is moderate in carbohydrates and low  in saturated fat, with a focus on balanced meals is recommended. In addition, patients with type 2 DM often require caloric restriction to promote weight loss. Bedtime and between-meal snacks are not usually needed if pharmacologic management is appropriate.

Aerobic exercise can improve insulin resistance and glycemic control in most patients and may reduce cardiovascular risk factors, contribute to weight loss or maintenance, and improve well-being. Exercise should be started slowly in previously sedentary patients. Older patients and those with atherosclerotic disease should have a cardiovascular evaluation prior to beginning a substantial exercise program.

CLINICAL PRESENTATION of DIABETES MELLITUS

Type 1 Diabetes Mellitus

  • Individuals with type 1 DM are often thin, prone be diabetic ketoacidosis if insulin is under condition or severe stress.
  • 20% to 40% of patients diabetes mellitus presents with polyphagia, polydipsia, polyuria, and weight loss after several days.

Type 2 Diabetes Mellitus
  • Asymptomatic, but presence of complication may indicate they have had diabetes mellitus for several years.
  • Lethargy, polyuria, nocturia, and polydipsia. weight loss is less common.

HOW TO DIAGNOSE BY YOURSELF?
You can buy and use a tester "Blood Glucose Tester" which can be purchased at drug stores. Its use is pretty easy. Typically use only a little blood sample at your fingertips. Just wait 5 seconds than the results will appear.
This tool can only be used to test for type 2 diabetes mellitus. Recomendation for normal condition are :
1. normal fasting blood sugar is less than 100 mg/dL
2. postprandial blood sugar is less than 140 mg/dL
3. glucose tolerance is less than 199 mg/dL

Diabetes Mellitus

DEFINITION
Diabetes Mellitus (DM) is a group of metabolic disorder characterized by hyperglycemia and abnormalities in carbohydrate, fat, and protein metabolism. It results from defects in insulin secretion, insulin sensitivity, or both. Chronic microvascular, macrovascular, and neuropathic complication may ensue.

PATHOPHYSIOLOGY

  • Type I Diabetes Mellitus accounts for 5% to 10% of all diabetes case. It generally develops in childhood or early adulthood and result from immune mediated destruction of  pancreatic B-cell, resulting in an absolute deficiency of insulin. There is a long preclinical period (up to 9-13 years) marked by the presence of immune markers when B-cell destruction is thought to occur. Hyperglycemia occurs when 80% to 90% of B-cells are destroyed. There is a transient remission ("honeymoon" phase) followed by established disease with associated risk for complication and death. The factors that initiate the autoimmune process are unknown, but the process is mediated by macrophages and T lymphocytes with circulating autoantibodies to various B-cell antigens (e.g islet cell antibody, insulin antibodies).
  • Type 2 Diabetes Mellitus accounts for as many as 90% of DM cases and is ussualy charactherized by the presence of both insulin resistance and relative insulin deficiency. Insulin resistance is manifested by increased lipolysis and free fatty acid production, increased hepatic glucose production, and decreased skeletal muscle uptake of glucose. B-cell dysfunction is progressive and contributes to worsening blood glucose control over time. Type 2 DM occurs when a diabetogenic lifestyle (excessive calories, inadequate exercise, and obesity) is superimposed upon a susceptible genotype. <Hg>
